How to Apply for TRICARE

Beneficiaries must complete TRICARE enrollment before they may obtain medical and dental benefits under a TRICARE medical insurance plan.

In many cases, applicants can sign up for TRICARE online, by mail or telephone, depending on the specific type of coverage they qualify for or wish to obtain. However, TRICARE beneficiaries do not need to complete the enrollment process if they receive medical benefits under Medicare Part A and B and qualify for enrollment in TRICARE for Life. If veterans and their families register in the Defense Enrollment Eligibility Reporting System (DEERS), their TRICARE for Life coverage is automatic.

Moreover, the process of how to enroll in TRICARE varies by plan type. For instance, applicants may choose to enroll in TRICARE Prime or Prime Remote online, by mail or telephone, but they cannot apply for Prime Overseas online. For some plans, applicants may apply by fax or in person. To learn more about how to apply for different types of TRICARE coverage, review the sections below.

TRICARE Enrollment Form Documents

Before signing up for TRICARE, applicants must gather information about their military sponsor and each additional family member they wish to enroll. For instance, applicants typically need to provide the name of the plan they wish to enroll in and information such as the following:

  • Sponsorship information. This includes the military sponsor’s date of birth, Social Security Number (SSN), military status, mailing and residential address and contact information. Applicants must also provide information about the sponsor’s military assignment.
  • Family information. If completing a TRICARE application for any additional family members, applicants must provide the name, date of birth, contact information and the mailing and residential addresses of each additional family member. However, the name must match the information in DEERS.
  • Other health insurance information. If any TRICARE enrollee qualifies for another type of medical coverage, applicants must provide information about these policies as well. For instance, applicants must provide the name of the person who is eligible for coverage, the name of the policyholder and the policy number.

When completing the TRICARE enrollment process, applicants may need to provide a signature, their relationship to the military sponsor, and the application date. Retired military members, surviving spouses and children of retirees, and eligible former spouses of servicemembers must also provide payment information for covering their enrollment fees. In most cases, applicants may pay by credit card, check, money order, or electronic funds transfer. Additionally, enrollees may pay monthly, quarterly or annually.

How to Apply for TRICARE Enrollment Online

While the process of enrolling in TRICARE varies by plan type, most TRICARE eligible applicants may apply through the Beneficiary Web Enrollment system under the website of the Defense Financial and Accounting Services (DFAS). Moreover, online enrollment is available to applicants who wish to enroll in the following plans:

  • TRICARE Prime
  • Prime Remote
  • TRICARE Select
  • The US Family Health Plan
  • TRICARE Reserve Select
  • TRICARE Retired Reserve
  • TRICARE Young Adult

To complete the online TRICARE enrollment process, applicants can get started by clicking on the red “Log On” button at the top of the page. Applicants must then log in to the system using their DFAS MyPay login ID and password, information from their Common Access Card (CAC), or they may use their Department of Defense (DOD) Self-Service Login credentials. After logging into the system, they must click on the “Medical” tab to complete the enrollment process. To apply for dental coverage, however, applicants must select the “Dental” tab.

Note: If TRICARE enrollees cannot provide the required login information, they must use an alternate enrollment method.

How to Submit a TRICARE Application by Mail, Fax or Telephone

While online TRICARE enrollment is not available for all types of plans, any applicant may enroll by mail, and some enrollees may be able to apply by fax or telephone. For instance, veterans and their families who wish to enroll in the following plans may complete the application process by mail:

  • TRICARE Prime
  • Prime Remote
  • Prime Overseas
  • Prime Remote Overseas
  • TRICARE Select
  • TRICARE Reserve Select
  • TRICARE Retired Reserve

Applicants may sign up for TRICARE Prime, Prime Remote, Prime Overseas, or Prime Remote Overseas by completing an East, West or Overseas Enrollment Form (DD Form 2876-1, 2876-2, or 2876-3). Three Young Adult TRICARE applications are available to the qualifying adult children of service members that are no longer eligible for other VA health care benefits for family members. The Continued Health Benefit Program

To enroll in the TRICARE Continued Health Care Benefit Program (CHCBP), applicants must complete a Continued Health Care Benefit Program Enrollment Application (DD Form 2837) within 60 days of losing their regular TRICARE eligibility. Then, they must mail the form to the following address along with proof of their CHCBP eligibility:

The US Family Health Plan

To submit TRICARE enrollment forms for the US Family Health Plan, enrollees may mail their information to the address noted on their application, or they may contact 1-800-748-7347 to be transferred to the office of their local USFHP site. Can you submit a TRICARE enrollment form in person?

In some cases, applicants may sign up for TRICARE in person at a local TRICARE Service Center. In most cases, however, this application method is only available to Reserve members who wish to enroll in TRICARE Retired Reserve from overseas.

How to Sign up For TRICARE Dental

To complete the TRICARE Dental enrollment process, applicants may complete the process online through the DFAS website, or they may call 844-653-4061, 844-653-4060 or 717-888-7400 to enroll by telephone. To enroll by mail, however, applicants must complete a TRICARE Dental Program Enrollment/Change Authorization and submit it along with payment for their first monthly premium.
